POSITION DESCRIPTION:
The Health Planning Council seeks a Manager of Healthy Communities. This is a newly created leadership position, reporting to the Executive Director and is responsible for the direction and development of the Health Planning Council’s Healthy Community Initiatives. The position is structured with a base salary along with a tiered business development bonus structure. The Manager will work across cross-sector organizations such as the public health systems, hospitals, city and county governments, FQHC, philanthropic organizations, and non-government organizations to develop, manage and facilitate community-based participatory research initiatives; M.A.P.P Health Needs Assessments, Strategic Prevention Framework assessments, community convening events, and manage the Northeast Florida Counts health-related quality of life indicator website. The Manager will facilitate or lead special projects relating to the development and implementation of public health or healthcare prevention initiatives. The Manager will also be responsible for designing methodology for surveys and focus groups and generate reports from qualitative or quantitative data.
MAJOR DUTIES:
• Consult and collaborate with a wide array of organizations, committees, groups and individuals to develop linkages, identify needs, problem solve, plan solutions, develop programs, and enhance leadership to address individual or community health concerns and enhance the community capacity for response.
• Maintain and update the HPC community health assessment policy, procedures, and process manual
• Develop, manage and update the Northeast Florida Counts health-related quality of life indicator dashboard
• Manage and Coordinate the Northeast Florida Counts Advisory Council
• Plan and conduct a variety of health needs assessments using qualitative and quantitative assessment methods, analyze results, write needs assessment reports, and communicate findings to key audiences
• Provide community education and awareness to the community and neighborhood organizations, and the local health care industry to address the Northeast Florida region’s health priorities and utilization of Northeast Florida Counts.
• Develop White Papers, fact sheets, and action alerts health-related quality of life issues, prevention strategies and policies, rural health, safety-net issues, Medicaid Reform etc….
• Develop, prepare, and coordinate fee-for-serve and grant applications and grant-related activities to obtain funding for health education programs and related work
POSITION FOCUS:

REPORTING RELATIONSHIPS:
Reports to the HPC Executive Director
Will work closely with the HPC Data Manager, Community Planner, and Director of Healthy and Happy Places.
KNOWLEDGE, SKILLS, ABILITIES, AND PERSONAL CHARACTERISTICS:
Must have a positive and energetic attitude; Possess solid organizational skills, writing, and communication skills; Must have the ability to speak in front of large groups and be a transformational leader. The Manager of Healthy Communities must be technology savvy; Demonstrate a history of superior customer service; Manage multiple tasks; Ability to work effectively with staff, partners, Board of Directors, and the public. The position should be familiar with database programs, Word and Excel, electronic survey programs, SPSS, social media and electronic communication platforms.
MINIMUM EDUCATION AND EXPERIENCE:
Bachelor degree in health planning, health science, health promotion, public health, or healthcare marketing. Have at least five years of combined experience in community health, health promotion, health marketing. Preference will be given to candidates who can demonstrate effective experience conducting community needs assessments.
