New Town Health Project Phase 5 and Assessment Summary
The Health Planning Council of Northeast Florida completed their partnership with Baptist Health and New Town Success Zone by facilitating Phase 5 in this groundbreaking community-based participatory research project occurring in the New Town census track. Back in April, over 23 trained New Town Health Project Health Builders! collected 665 community surveys. In May, Health Builders! gathered on the campus of Edward Waters College to review the survey information and developed four core recommendations. This is their video diary.
