NASSAU COUNTY OPIOID CARE CONNECT – RURAL COMMUNITIES OPIOID RESPONSE PLANNING NEEDS ASSESSMENT

 

The Health Planning Council of Northeast Florida (HPCNEF), in partnership Starting Point Behavioral Healthcare (SPBH), St. John’s River Rural Health Network, Florida Department of Health in Nassau County and the Nassau County Care Connect Consortia spearheaded a needs assessment to identify Nassau County’s strengths, opportunities, barriers and gaps in the continuum of care associated with Opioid Use Disorder/Substance Used Disorder (OUD/SUD). The goal was to identify how the areas of OUD/SUD prevention, treatment, and recovery services can increase capacity, improve accessibility and effectiveness services while reducing barriers. Yulee, FL –NASSAU COUNTY OPIOID CARE CONNECT CONSORTIA MEETING

 

Community partners, stakeholders, and members of the community are invited to join us on Thursday, October 10th from 1:00 – 3:00 at the Christ Fellowship Church’s Fellowship Hall, 850987 US Highway 17, Yulee, for the first Nassau County Care Connect Consortia Meeting, a planning meeting dedicated to fighting the opioid epidemic in Nassau County.

 

The Rural Communities Opioid Response Program-Planning Grant (RCORP) supports a planning partnership between the Health Planning Council of Northeast Florida (HPCNEF), the St. John’s River Rural Health Network, Starting Point Behavioral Health, and the Florida Department of Health Nassau.  The partnership will seek solutions in the treatment for and prevention of substance use disorder, including opioid use disorder, in rural counties at the highest risk.

 

Partners will share the needs assessment and service gaps related to Nassau County. Nassau County Care Connect will seek additional community participation from stakeholders, residents, and volunteers interested in participating in this important community-wide effort in creating a strategic plan to address the opioid crisis in Nassau County.

 

Additionally, the Community Coalition Alliance (CCA) in collaboration with LSF Health Systems and the North Florida HIDTA will leverage this meeting opportunity to engage stakeholders in a discussion around needs and existing gaps for community-level prevention efforts to address substance misuse. Information will be shared across key data indicators, the results of the community feedback survey, and available strategies to identify community-level prevention priorities in addition to opioid misuse and abuse.

 

We are asking community partners, stakeholders, and members of the community to complete the online survey, as it will be instrumental in identifying current needs to support community efforts on combating substance misuse.

Yulee, FLNASSAU COUNTY OPIOID RESPONSE PLANNING INITIATIVE KICK-OFF EVENT

 

Join Judge James Daniel on Tuesday June 25th from 4:30 – 5:00 on the steps of the Nassau County courthouse, located at the Judicial Complex, 76347 Veteran’s Way, Yulee, for the official kick-off of the Nassau County Opioid Response Program Planning Initiative (ORPPI-Nassau).  This initiative is funded through a grant from the Health Services and Resources Administration (HRSA), an agency of the U.S. Department of Health and Human Services (HHS) to assist rural communities responding to the opioid crisis.

 

The Rural Communities Opioid Response Program-Planning Grant supports a planning partnership between the Health Planning Council of Northeast Florida (HPCNEF), the St. John’s River Rural Health Network, Starting Point Behavioral Health, Nassau Alcohol Crime Drug Abatement Coalition (NACDAC), and the Opioid Task Force.  The partnership will seek solutions in the treatment for and prevention of substance use disorder, including opioid use disorder, in rural counties at the highest risk.

 

From 2007 to 2017, the rate of unintentional injury deaths by drug overdose in Nassau County more than doubled from 16.7 to 34.0 respectively. The overall goal of this initiative is to reduce the morbidity and mortality associated with opioid overdoses in high-risk rural communities through multi-sector consortiums and strengthen organizational and infrastructural capacity.

 

According to Susan Grich, President and CEO of HPCNEF, “opioid use disorder is a national epidemic and HPCNEF is committed to help the rural communities in Nassau County develop a solid program to reduce the incidence and prevalence of opioid use addiction. We are extremely pleased to have the support of Congressman John Rutherford.”

 

“As a former sheriff, I have seen firsthand the opioid crisis and its heartbreaking impacts, especially on our rural communities,” said Congressman Rutherford. “Solving this crisis won’t be easy – it will require teamwork among state, local, and federal partners to find solutions and address the specific needs of each community. I am pleased to see Northeast Florida receive these additional federal resources to combat a growing epidemic, and I pledge to continue supporting these efforts in Washington.”

Jackson County Community Health Assessment and Improvement Plan

The Health Planning Council of Northeast Florida (HPCNEF) collaborated with the Florida Department of Health in Jackson County on their countywide Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). During these meetings, HPCNEF facilitated the creation of the goals, objectives and strategies for the top community health issues, which were identified using data and community input: 1) behavioral health including substance abuse and 2) chronic disease and healthy lifestyle. Jacksonville Nonprofit Hospital Partnership Community Health Needs Assessment

 

The Community Health Needs Assessment (CHNA) is conducted every three years to provide local health care organizations and health departments with an accurate understanding of evolving local health needs. The Jacksonville Nonprofit Hospital Partnership commissioned the report. The group’s vision is to improve health in the region by addressing gaps that prevent access to high-quality, integrated health care and by improving access to resources that support a healthy lifestyle.

 

CHNAs seek to identify significant health needs for particular geographic areas and populations by focusing on these questions:

 

  • Who in the community is most vulnerable in terms of health status or access to care?
  • What are the unique health status and/or access needs for these populations?
  • Where do these people live in the community?
  • Why are these problems present?

This was the third CHNA study conducted since the formation of the partnership in July 2011 by leaders from Ascension St. Vincent’s, Baptist Health, Brooks Rehabilitation, the Clay County Health Department, the Duval County Health Department, Mayo Clinic, the Nassau County Health Department, the Putnam County Health Department, and UF Health Jacksonville.

Job summary

The Health Planner fosters and leads initiatives to assess population health and develops plans and programs that address area health needs. Responsibilities include developing and implementing community health assessment and planning cycles. The Planner will identify, develop, and execute health planning, research, and the evaluation of projects in the seven county service area comprised of Baker, Clay, Duval, Flagler, Nassau, St. Johns and Volusia counties, consistent with Florida State Statute 408 033..
