Reducing emergency room over-utilization and building healthier communities are at the core of the Health Planning Council of Northeast Florida’s efforts. That’s why much of what we do supports the design and management of disease prevention programs to provide alternatives to emergency room utilization and to promote wellness.
That’s also why we research, develop, execute and evaluate disease prevention / management programming such as HIV/AIDS patient and housing services as well as flu, pneumonia and hepatitis vaccination programs for high-risk minority populations. These programs are provided through houses of worship, community centers and homeless shelters, among others.
Using innovative technology, we initiate health communication campaigns that target legislators and local government officials, social service safety net providers and the public at large to raise awareness of health injustices identified through in-depth consumer research and community-needs assessments.
