The Health Council completed county health profile brochures for each county within District 4 which includes; Baker, Clay, Duval, Flagler, Nassau, St. Johns and Volusia counties. These county profiles highlight key demographic, socioeconomic, and health care utilization data. The profiles have been distributed to community leaders, health care administrators, elected officials and other interested parties throughout the northeast Florida area. Copies are available by calling the Jacksonville office (904) 723-2162 and they can also be viewed and downloaded from our website by clicking on the county link listed below.
