The Health Planning Council of Northeast Florida is the repository of healthcare and health status data for Baker, Clay, Duval, Flagler, Nassau, St. Johns and Volusia counties. The Health Planning Council collects psychographic data by zip code, conducts qualitative and quantitative data, analyzes social determinant data by state and county in addition to collecting and analyzing utilization data submitted by hospitals and nursing homes.
We offer healthcare utilization subscription services to more than 250 subscribers who receive our healthcare utilization reports monthly, quarterly and semi-annually. We also generate customized utilization reports on request. Please contact us for more information.
