Region IV Health Plan 2003 - 2006

The Region IV Health Plan was prepared by the Health Planning Council of Northeast Florida as
part of its responsibilities under Florida law and under contract with the Florida Department of
Health. The Health Planning Council of Northeast Florida is part of a network of eleven local
health councils throughout Florida. Regional health plans are developed by each council to
evaluate and identify local health care needs and recommend ways to meet identified needs.
 

 

Guide to Assisted Living Facilities

     The Health Planning Council of Northeast Florida produced the third edition of the "Guide to Assisted Living Facilities in Northeast Florida 2007 - 2008". The Guide is designed to help consumers and their families make informed decisions on the type of long-term care that is needed via providing information about the different kinds of assisted living facilities available, the services they provide, how to select a facility that meets their needs, as well as additional information on other alternatives to long-term care.    ALF Guide 2007 - 2008.pdf

Mission

The vision of the St Johns River Rural Health Network is to recognize, encourage, and support partnerships to improve the health of communities in Baker, Clay, Flagler, Nassau, Putnam,             St. Johns, and Volusia Counties.
 

The St Johns River Rural Health Network (SJRRHN), Inc. is a 501-C3 not-for-profit Florida Corporation established in 1994.  Located in the northeast corner of Florida, the St. Johns River Rural Health Network consists of Baker, Clay, Flagler, Nassau, Putnam, St Johns, and Volusia Counties.

Three counties are designated Rural Areas (Baker, Nassau, Putnam) and all counties are either designated as Health Professional Shortage Area (HPSA) or Medically Underserved Area/Medically Underserved Professional, and/or Low Income. An estimated 907,469 persons live in the network area, according to the 2000 Census.

The area’s population has increased approximately 20% between 1990 and 2000. The percentage of uninsured for all counties range from 12.63% to 18.85% and the percentage of those living 150% below the federal poverty level ranges for 14% to 34%.

The adequacy of rural health care services, in terms of accessibility, quality, availability, and affordability, is fundamental to the health of a community. Many of the residents who live in the SJRRHN area must travel far distances in order to access acute care hospitals, primary care, and mental health and substance abuse services. Access to health care remains an issue of growing concern for residents living in rural communities in Northeast Florida.
 

Membership

The SJRRHN members include representatives from all seven county health departments located in the network area, representatives from several of the hospitals, medical providers, mental health providers, community agencies, employers, as well as a variety of social service agencies.

The SJRRHN board includes officers who are elected on an annual basis by members of the network.

 

Meetings

Regular meetings are held on the 1st Thursday of every other month (Jan, Mar, May, July, Sep, Nov) at 1:00 pm at the St. John’s County Health Department. Presidents from each of the regional rural health networks also meet on a quarterly basis with the Florida Department of Health.
 


 

              FCCPP

FIRST COAST COMMUNITY AIDS PREVENTION PARTNERSHIP                                          

The Health Planning Council of Northeast Florida provides technical assistance and staff support to the First Coast Community AIDS Prevention Partnership (FCCAPP). First Coast Community AIDS Prevention Partnership is a group of committed individuals from the greater Jacksonville area that includes the counties of Baker, Clay, Duval, Nassau, and St. Johns who have come together to combat the spread of HIV/AIDS.  FCCAPP is a partnership between the Centers for Disease Control and Prevention (CDC), the State of Florida Department of Health, and the community. FCCAPP’s purpose is to develop the HIV Prevention Plan that addresses the HIV prevention priorities for the area, and then defines the types of prevention programs that are necessary to address the established priorities.

FCCAPP Meetings are held the first Tuesdays of every other month (February, April, June, August, October, December) at 5:30pm at the Duval County Health Department, Jacksonville Central Health Plaza, 515 West 6th Street.

PCHP

PARTNERSHIP for COMPREHENSIVE HIV/AIDS PLANNING

MISSION

To plan a comprehensive array of HIV/AIDS services spanning from prevention to early intervention and patient care through active, open, and inclusive community planning processes that emphasize delivery of quality and effective services to all clients and communities affected by HIV/AIDS within the boundaries of Volusia and Flagler Counties.

The Health Planning Council of Northeast Florida provides technical assistance and staff support to the Prevention and Education Planning Partnership of Volusia and Flagler Counties.

What is PCHAP?

PCHAP represents Volusia and Flagler Counties, or Area 12 that is one (1) of seventeen (17) local planning partnerships within the State of Florida. It is a planning body or consortia comprised of state government employees, representatives from community-based organizations, AIDS service organizations, and members of the community who are affected by HIV and AIDS. Members of PCHAP collaborate to develop a comprehensive plan for HIV/AIDS prevention, early intervention, and patient care that is consistent with the priority needs identified within our communities.

Among its activities, the planning body performs needs analyses, assessments of local resources, identification of priority populations with service gaps, allocation of funding and resources, and evaluation of effectiveness for the planning process. It also continuously works toward improving the accessibility, quality, and effectiveness of the HIV/AIDS related services that are available within the two (2) counties.

How to Become Involved...

PCHAP offers individuals an opportunity to voice their ideas and concerns about how prevention and care efforts are coordinated in your area. Parity, inclusion, and representation are fundamental tenants of HIV/AIDS community planning. Parity is when all members of the community planning group have the opportunity to acquire skills and knowledge to participate in the planning process and have equal voice in voting and decision-making. Inclusion is the assurance that the views, perspectives, and needs of all affected communities are included and involved in a meaningful manner in the community planning process. Representation is the assurance that those who are representing a specific community truly reflect that community's values, norms, and behaviors. Representatives from our local communities are needed to provide insight and varying points of view. You can become involved by: (1) Becoming a community member by attending meetings to provide opinions, ideas, and suggestions; or (2) Applying to become a voting member of PCHAP who represents a specific community or area of experience.

DUVAL COUNTY IMMUNIZATION TASK FORCE


MISSION

TO DEVELOP AND IMPLEMENT COMMUNITY WIDE CAMPAIGNS THAT WILL IMPROVE IMMUNIZATION RATES FOR CHILDREN AND ADULTS LIVING IN DUVAL COUNTY.
